I've never seen one that looks like that, mine is this style

http://www.ebay.co.uk/itm/Cumana-CAX-354-Disk-Drive-for-Commodore-Amiga-2-/400368911734?nma=true&si=725%252FiZMxrN2b6H7Uj8%252BzGm7ZfhA%253D&orig_cvip=true&rt=nc&_trksid=p2047675.l2557

Later they did http://www.retro-kit.co.uk/page.cfm/content/Cumana-CAX-354-Floppy-disk-drive/

They are both quarter height drives, but the black one likes half height (although it might be the angle of the photo).

The face plate looks like an Atari Drive. Here is one with a very similar face.

I know the make and model of the drive... but I'm curious as to what company assembled the case, and made the PCB adapter that appears to be required to use it on an Amiga ;)

Actually I don't know about your drive a thing, but as a matter of fact I have this ugly beast lying around, which is almost same looking front cover and I was thinking the same, what is this came from (It's TEAC made as I can see)? As I tested on one of my Amigas it worked as internal drive, but when it arrived to me, it had handmade steel external case with mucho wires going everywhere inside it, very very hacky stuff, so I discarded this case. This seems very old...

Nope, this drive isn't a TEAC ;)

Obviously the mechanism is, could it be Skai?  They rebadged a lot of stuff mid 80s to early 90s.
