How come some icons (like the latest AHI) can never find the tool they are associated with even if it's right where it should be?

1.Perhaps some standard paths/assigns are missing?
2.Maybe the associated tool can't be read as a tool, ie it has faulty bits set or perhaps actually are project-files instead of .exe/tools?
3.Could be they aren't located where they should be?

Hard to tell without having the problem in front of me :p
Try running DOS-Trace and see what happens when the WB tries to open them.

Yeah, I think you're on the right track with that answer.  I have used snoopdos more than a few times to figure out what libs/tools are missing.  I remember vividly getting my IRC client to work and I had to use snoopdos to figure out what was missing.
