Works great, with a minor cosmetic issue.
When I got it, the top cover was missing. It now has a cover from an Impact II, same one with different stickers. (Sold just the card from the Impact II to a cool guy in Sweden, hence the extra.) Photo of what it looks like
here. 4MB installed, 50MB Quantum (IIRC), and I'll also include a 50MHz 68882. (Card runs at 40MHz.)
