Verify orientation of the power connector and verify the card is seated properly.

Make sure you are using the latest diagnostic software as some older revisions will not work with the latest firmware.

BTW: prometheus is not crap my any stretch of the imagination.

Yeah, does sound like a problem with the Amiga itself :-/ Maybe try removing the daughter board and cleaning the contacts :-?

Do you happen to have a service manual for the A4000? It has been a long time for me, but I seem to remember the A3000 having multiple fuses on the motherboard and I'd assume the A4000 is the same. They don't don't look like your typical glass tube fuses, so a service manual to reference would be helpful. I'd also suggest testing power supply voltages.
