Did you connect a 15khz monitor to the aga output or a vga/multiscan monitor? The csppc has a feature to show everything (csppc menu/early startup menu/etc) in 31khz. Maybe this feature is switched on. You can set it in the configuration menu of the card (hold esc after power on/reset).

Hmm I dont know, I have bought one at a local amiga dealer some years ago. And a few weeks ago I have seen one at ebay.de. But I think the easiest thing to do is to reflash the csppc so it will have the default settings. You will just have to extract the archive, copy the file to a floppy disk or ide drive,  boot without startup sequence and execute the flashupdate.

I dont know if its the latest flash update (I also got my first csppc this month) but i think so (who other than dce should have the latest version?). I hope it helps.

Oh, I didnt know that. So no need to flash the card because of the screenmode. :-)
