No luck for me there, unfortunately.  I've done quite a bit of searching and this particular install disk really needs to be archived for posterity (and owners like me!).

Thanks for the link - that has to be one of the most funky pieces of Amiga hardware ever! Probably beats out Jens' X-Surf ISA-board-soldered-to-Zorro contraption.

I doubt there even exist any of those adapters on this side of the pond - it's sort of a roundabout way of cobbling together a Bridgeboard. Cool!
