Only available from Newtek (sold by Commodore to Newtek and configured by them for resale)? Or did Newtek just sell the boards with the stickers to cover up the Amiga name?

Yes.  What is the difference?
Wanna try a wonderfull strategy game with lots of handdrawn anims,
Magic Spells and Monsters, Incredible playability and lastability,

I thought they were sold by Newtek, someone else told me there was no such thing as a 4000 sold by Newtek. Not that going to lose much sleep over it :-).

I believe newtek sold them as a bundle as their customers likely wouldn't know what an Amiga was, they just knew they needed a video toaster.
 
There seem to be a variety of stickers, although that might have been cut down.

It's rumoured that newtek produced the larger stickers because they hated commodore and didn't want them to get any publicity. However I imagine it was to remove any stigma of having a failed computer as the basis for their product.
 
After commodore went bankrupt newtek must have had a stash of a4000's somewhere. They possibly even had to refurbish them.
 
Maybe Wil Wheaton was just being evil (after star trek tng he went to work for newtek as a tester).
