Only available from Newtek (sold by Commodore to Newtek and configured by them for resale)? Or did Newtek just sell the boards with the stickers to cover up the Amiga name?

I believe newtek sold them as a bundle as their customers likely wouldn't know what an Amiga was, they just knew they needed a video toaster.
There seem to be a variety of stickers, although that might have been cut down.
http://www.amiga-hardware.com/display_photos/amiga4000_2_sm.jpg It's rumoured that newtek produced the larger stickers because they hated commodore and didn't want them to get any publicity. However I imagine it was to remove any stigma of having a failed computer as the basis for their product.
After commodore went bankrupt newtek must have had a stash of a4000's somewhere. They possibly even had to refurbish them.
Maybe Wil Wheaton was just being evil (after star trek tng he went to work for newtek as a tester).
