HDtool box error while Read Drive Configuration

So...
After a couple months of leaving the A3000/Warpdrive'40/PicassoII 3.5os (4.5 gig hd, external CDrom, Zip drive and Scanner, running off the stock a3000 scsi contoller ) alone cuz it torques me sooo much when I go to use it, I decided to fire the system up to try and produce some logos with Lightwave. Cleared off the desk, started the system and left the room to answer the phone. Returned to the computer room about an hour later and shut down the computer. Next day went to start it all up again and got the DH0: Not a DOS Disk error. All other partions OK.

sigh... Remembered once again why I bought a new HP Athlon XP "does everything out of the box" system.

As a glutton for punishment, I rebooted to the backup 3.1 os partion with the hopes of fixing the primary boot partion.  Then came time for dinner and I shut the whole thing down untill I had more time to dink with it.

Couple days later, boot to the back up partion and get DH0: Not a DOS Disk error on the backup 3.1 partion. All other partions ok. Dug out the Install disks to do a complete rebuild of the HD only to get an error while reading the Drive Configuration.  Trying to input the Drive Configuration by hand but am a bit miffed on figuring out the proper numbers. (Been a while since I've done this...)

Here's the drive specs for the Seagate ST 34520N

Need to know:
Cylinders = ?
Heads = 4
Blocks per Track = ?
Blocks per Cylinder = ?
