These faults are familiar to me. Try the following lower your Transfer mask to 00x001FE00 in HD toolbox and install or copy the game again. Now it should work.
If it doesn't work try an older WHDLoad version. You could find in the readme of Pang which version of WHDLoad it at least needs.
Sometimes the 16.9 version can have problems with older installation of WHDLoad games.
