I have the same card and they are correct that P4 is NOT for HD power.

Did you try to move AUTO jumper to NO position?
Mine for some reason, can not AUTOBOOT, it only works when booted from floppy disk first.

Are you able to make disks from ADF?
If you do, I can make you copy of my Supra boot disk that allows to boot from floppy and then continue to  boot using the HD.
Worked fine last time I used it. I'm pretty sure I can find it somewhere.

I also would recommend that you should find yourself another SCSI controller, because this one sucks. Commodore A2091 is way better and has some Fast Ram on board too.
