I was at the post office once and some lady was making a big scene because she was trying to send medical records without any binders. The rules for media mail say something like "loose papaer and their binders" and her contention was that it doesn't say that the papers had to be in binders.
That was the first tim eI ever heard of the post office opening boxes to check if they qualify. I guess it depends on the city. In my old city, they didn't open nothing.
