Get a VGA cooler if you don't already have one. It will give you some peace of mind :-)

You don't need to. Simply mount an appropriately sized fan on to the existing one. I have found in the past that the right length screws can screw down into the heatsink and hold the fan tight in place.

You'll probably need to leave a PCI slot free to make room for this.

Also make sure you have good overall airflow in your case. Often it's good to put a case extraction fan just above where the cards sit (usually inline with the location of CPU on an ATX motherboard) assuming your case has provision for this.

You'll find the glue it is attached with doesn't really soften much as it gets hot.

Crazy as it sounds, Speel is right - but you have to be very careful. The glue is brittle when cold and you can generally remove a heatsink with a chisel. I removed my old 040 card's pathetic heatsink and fan this way.

BUT: I wouldn't try this on a gfx card heatsink! The chances are that the BGA connections would come undone before the heatsink.

Ok cool. Can't have been the same stuff that was on my 040 card then. That was like cement, no matter how hot it was ;-)

You might want to ensure you clean any of the glue thats left off the chip and put a bit of thermal transfer compound on.
