Yeah, the mouse look mode was off by default on original PC Duke too.  I don't know what people are talking about it being essential part of the game.  On original old PC back in the day it was slow too.   Most people I know never used it.  A lot of people never knew you could turn it on at all by pressing "U".  

On the Amiga version you are playing if you press "U" does it turn mouselook mode off by chance?
