The cs mk2 scsi has built in termination (automatic), so your old setup, controller-hd-cdrom will work just fine.
As for firmware (if cybscsi.device does not show up), you're looking for a file called 'Mk2update' (note the '2' indicating a cs mk2).
Hint:
Try the following CLI command:
version cybscsi.device

If a version number shows up, everything is fine, if you get a complaint about 'no such device' you will need to upgrade your firmware.
I think I have the latest mk2 firmware lying around at home somewhere on my hd, I can mail it to you when I get home on friday.

The firmware update i have is most likely the same as yours.

Question: Does your amiga boot from the cs mk2 scsi? try this... It should work now. Regarding the A4091, try moving it around to a different zorro slot, I had the same problem myself with a fastlane z3 that got cured once I moved it to another zorro slot.

Do note this: Some cs mk2 scsi modules do not work on overclocked cs mk2's. If you have an overclocked cs mk2, then drop back to 50 MHz again.
The modules that do not work on overclocked boards have a ncr scsi chip, where as the ones that work with overclocking have a FAS216 scsi chip.

Jumper settings:
jp0: debug mode, leave open
jp1: reserved, leave open
jp2: slow inquiry mode, default=off=open
jp3: slow cable mode, default=off=open
jp4: synchron auto-enable, default=on=closed
jp5: reserved, leavee open

Settings for the termination jumper on the external scsi connector pcb:
1-2: Auto termination: Default
2-3: Termination forced OFF
off: Termination forced ON

Question:
Have you remebered to change the device name in your cd0: driver to cybscsi.device?

Regarding speed, the cybscsi is not that much faster than the A4091, I personally ditched the cybscsi, overclocked to 66 MHz and use an A4091.
