Isn't there some dentist using 4.1 on an AmigaOne somewhere?
I think Ferrule Media uses AmigaOS 4.1 and SAM machines from Acube to power their "Dental Kanal" software (
http://www.ferrule-media.no/dental-kanal.html). There should be some dentists around using this software.
