Quake2 itself is a shell program which can only be run from the command line. Therefore QuakeIIGUI was created which opens a window and allows you to change options comfortably.

So yes, if you double click QuakeIIGUI, a window should pop up before the game starts. You might want to run SnoopDOS in order to check for missing libraries. You are probably missing wizard.library. Another reason could be that there are some settings which tell QuakeIIGUI to immediately run the game. Check the icon's tooltypes.

As for the person always running in circles, if you've got a Mroocheck, Cocolino, Topolino or other mouse adapter, disable the wheel driver before you run Quake2 (or Quake1 or Heretic2 or similar games). It interferes with lowlevel.library which is used to read the game controllers.
