Have you tried booting it with the hard drive disconnected and checking to see if the board shows up in early startup ? It could be that the A500 supply you have been using is giving up as the 040 draws a fair bit.
I had the same board but at 25mhz and when my Goliath power supply went belly up I tried an A500 power supply but it was a no go with everything plugged in.
