have you got the phase 5 060 libraries installed in libs?

check that the mark 3 is properly seated in the cpu slot.

have you moved the motherboard cpu jumper to int or ext ( forget which for a mark 3 board)

may be the mark 3 scsi is conflicting with the motherboard scsi when you install 3.9.  Why not put your hard drive and cd drive on the motherboard scsi and disconnect anything on the the cyberscsi interface, then install 3.9

i think the best advice at this point is to strip the A4000 down to the basic cpu card hard drive and cdrom, preferably with the hard drive and cdrom on the internal ide interface and try a clean install of os 3.9.  Once this works then add your peripherals one by one.  OS 3.9 does update the scsi.device, and there may be some confusion with the this update that causes a conflict with your existing scsi peripherals
