APlayer has always been my favorite
Does most formats and you can play along with the keyboard or just pause the mod and jam with the samples all by yourself :-D HippoPlayer I always considered a close 2nd (found the Gui a bit confusing at times).
As for Linux (for whoever was asking), XMMS has always done a decent job for me... There's always Deliplayer for Windows too.
