It's definitely a rare piece of card.
From a quick search on BBOAH it's probably the accelerator of the early stages of A4000 named A3400: http://www.bboah.com/index.php?action=artikel&cat=7&id=1859

As the article says:
- The A3630 is labelled "A3200/A3400 68020/030 BOARD Rev 1.0 "

It's exactly what the picture shows also :eek:

Probably some very early A4000 models must have gotten this accelerator card!
