Another thing to check would be the revision of your Buster chip. I've heard that CV643Ds get a bit unstable when running on an A4000 with revision 09 Buster chip. Solution in this case would be to upgrade to a revision 11 Buster chip.
If the above problem is what's wrong and you can't get a revision 11 Buster you could also try to force the card into Zorro II mode by way of a jumper on the card. Even if the Buster is not your problem it still wouldn't hurt to try Zorro II mode anyway to see what happens.
Word of warning: running the card in Zorro II mode will result in a performance penalty but if it solves your problem and you don't mind the performance then it may be okay for you.
