If it's just the two devices on the bus (controller + drive) at least one of them needs to provide termpower - both won't hurt - and both need to be terminated (resistor packs or termination chips). Any photos from the controlller?

Problems with termpower or termination won't damage anything - but the filesystem. ;)

Yes. Termpower that is, it's the only power distributed on the bus. Regular power needs to be supplied from the PSU.

So, you have zero problems using the HDD alone, floppy alone and both in combination? Then probably the CD drive is the problem. Tried copying some files to RAM:?
