Looks very familiar - I think I've got the very same one.
If the lights are always off, there's probably something wrong: A2065 damaged, transceiver dead, cable dead. Do you own an old PC NIC (e.g. 3C509) with AUI where you can test the unit? I've got a lot, they're free for postage.

These old transceivers are pretty cheap; might be the easiest way to just buy another one. $15 is pretty overpriced, I paid ~2€ for mine (eBay Germany, used).

Easily sufficient. I can't imagine it drawing much more than 100 mA.
