Indivision MK II Aga, Please help...

Just received one from Vesalia, first i plugged it into my "working A1200", with blizz 1230 IV, 4 way Ide and 4gb cf card, the picture was full of pink horizontal lines, then i removed blizzard, the same thing , after that i removed 4 way ide , the same thing , then i switch to original power suply , the picture was the same.
With that naked setup i plugged Amiga into lcd tv via vga port, the picture was the same, and the last thing i switch tho whole motherboard... you guess  the same thing...
Has anyone experienced similar problems??

Thank you for your replays, and yes i tried a fey times to remove it and to put it back, as i mentioned  i even tried to switch motherboards without success, the last I'm going to to today is to went to local shop and tried to find some other DVI-VGA adapter .

I attached screenshot of a boot screen, it is hard to capture lines with phone, but belive me the lines are very visible , especially when you boot in to workbench...
