You need one of these Mediator 1200 boards:

Mediator PCI 1200 SX, Mediator PCI 1200 LT4 and Mediator PCI ZIV

If you the plain "Mediator 1200" (it just says "Mediator 1200" on the board) the card won't be usable by AHI..it will show up inm PCIINFO but it won't be shown in AHI.PREFS
