sorry to dig this one up, I have a similar problem but with 060 card in A4000. also sometimes there's a flash of blue screen (custom chips bad). usually it gives guru 80000004. I've tried reseating cpu card and it sometime seems to make a difference. also, i used a lot of contact cleaner spray on power connector, hope that didn't break something.
what could it be?
are 040 libs and patches absolutely neccessary? because, sometime it booted OK without them.
If you're running an 060, having the replacement library is necessary. You might also want to look into OxyPatcher.
Other than that, make sure all your contacts are clean on anything that is socketed, not just the PSU.
