IIRC, it does.  

But, you can just delete the CD and/or CD0 files from DEVS/DOSDrivers and then remove or rename IDEFix in C:.  Then run setup again and choose the Add CD-ROM Support.

No.  The demo/shareware is from the shareware IDEFix'97 you used.
