The extra 4 pins might be the jumpers.  Check the part number on the Hitachi storage page to see if you can find drive information/specs.

I checked Usenet and found some more info.  Looks like grounding pins 47-48 should set the drive to slave mode.  I'm guessing the 45-46 is master, and no jumpers is cable select.  They also refer to a Thinkpad mailing list, but these messages were from the 90s so they're probably history.
