Check in the registry that there is nothing stuck in:

HKEY_LOCAL_MACHINE\Software\Microsoft\Windows\CurrentVersion\RunOnce

or

HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\RunOnce
