Could be a result of bad pressing. The best way to get to the bottom of this problem is to try and browse through the OS3.9 directories by inserting the CD into your PC's CD-R drive. If you don't get any trouble that would mean that the CD is OK.
Also try inserting some other CDs into your Amiga CD-R drives and see if you get similar error reports again or not.
