I have found an old SoundBlaster PCI128, and this can be found by the G-Rex, as well as my TV card. However, the Voodoo, Terratec 512i and ethernet can't.
Do you get a picture on your VGA monitor through Voodoo at all, or you simply can't since the card is not even detected in the PCI menu?
Heat wasn't an issue during testing, since the top has been off during the whole period. No harddrive have been connected during test, so the PSU should easily be able to cope with the accelerator, the busboard and 2 PCI cards.
I get the same errors with both a CSPPC rev. 2, and a CSMK3 rev. 4, so the busboard is going back for repair.
The basic test of a functional Grex 1200 or 4000D setup would be inserting the Voodoo 3 card only, and getting display (early boot menu or the CSMK3/PPC menu) on your VGA monitor through it. Assuming that the accelerator card's firmware has been updated to the appropriate flash that is. :-)
The only other thing that you could try perhaps is another A4000 motherboard, but this is a loooong shot.
I've had a similar problem to yours some time ago, but this was related to a faulty Grex 1200 board. Basically, cards would be detected in the PCI menu, but with rather strange info, such as Unknown Display Device or similar...and I'd get that for Voodoo 3 which is compatible. :-?
