WarpOS prefs is already in Prefs: (OS3.9)

Have you removed the ppc.library(Phase5) and installed the ppc.library EMUL?

WarpOS settings:

Termnator OFF
NoPatch

Use always address video= E0000000

from the Warp3D "Problems" readme:

Voodoo 3:

The Voodoo 3 driver is reported to work with the following revisions
of the graphics board drivers, and should work will all later versions:

Picasso 96:     Picasso96API.library    2.300
        rtg.library             40.3992
        3dfxVoodoo.chip         6.375

        Prometheus.card         6.51

CyberGraphics4: cgxsystem.library    42.5
        voodoo3 Monitor        4.3

To use the PPC driver in conjunction with WarpUp software, the following
environment variables should be set:
PowerPC/force 1
PowerPC/gfxaddr $xxxxxxxx

The installer will try to figure out the gfxaddr setting by itself. If it fails,
you have to do it yourself. The gfxaddr variable should be set to the Voodoo3's
graphics memory base address. The method to find this depends on the PCI bridge.
Consult your documentation on how to list the installed PCI boards.

nice those env prefs editor WOSPrefs . . .useful for WarpOS, StormMesa, GLUT and Warp3D too.

Better than the OS3.9 one.
