The original gizmo ;) just seems to hold the filters for the output, so the you will manage without it, if I am not totally wrong the filters are there to make sure the device doesnt interfere with other electronical equipment and can get the FCC-approval.  You should even get a clearer picture without the filters if you make a cable yourself and use ok cabling. Flat cable as used originally by Phase5 and DCE(?) aint really recommended for monitor-signals, they should actually coaxial-cables with an impedance of 75 Ohms for the Red, Green, Blue, Horisontalsync and Verticalsync signals.

Must just say one final thing. As the connector on the BlizzardVision only got 10 pins it is probably contains the five Red, Green, Blue, Horisontalsync and Verticalsync signals plus one ground for each of them, though I am just guessing, but it would not be hard to find out with a multimeter.

Pinout-guess (will look a bit wierd when submitted):

1         9

2          10

1 - Red
2 - Red Ground
3 - Green
4 - Green Ground
5 - Blue
6 - Blue Ground
7 - Horisontal Sync
8 - Horisontal Ground
9 - Vertical Sync
10 - Vertical Ground

Ok this is as stated just a guess. The ground-pins of the connector on the BVision are very easy to measure with a multimeter. Just set it to measure resistance in Ohms, connect one of the probes to a known ground on the card, put the other probe on a pin of the connector - if you measure a resistance of about 1 Ohm it should be a ground-pin.

The through-plated big mounting-holes on the BVision should be ground... cant guarantee it though, plug the card in the computer and measure if the holes are connected to the ground of the chassis with the above method to know for sure.

(edit) If you can get hold of a multimeter which can check connections and give a beep if connected this is even simpler :). Usually that selection is among the resistance-measurments on the multimeter and is marked with a diode-symbol.

You are right in that the 68040 is about the same size as a pentium.

An old Pentium-MMX I got here is:
49x49 mm

An old ;) 68060 (same size as 68040) I got here is:
47x47mm

It wont hurt, that much I can tell you, aspecially not if you are having issues with it (FUCKED UP?). Some PC card manufacturers making cards based on the Permedia 2 chip seems to think it is needed, check out the Diamond Fire GL 1000 card. Though you could always check if heat is the issue by letting a extra fan blow over the card.
