C-LTD controller + C-LTD Drive - How to format ?

I have a C-LTD A1000 scsi controller with a C-LTD hard drive (A box with a huge 5,25' MFM hard drive with a scsi adapter).

I never succeded to prep and format this drive (several weeks of work without success!)

I have tried utilities found on aminet (scsiltd1.2 and cltd utilities) but it's not the original utilities provided by C-LTD.

I have a KS 1.3 and a WB 1.3

Each time I try to format, prep, or make partitions i have a "Error 43" on write.

Mountlist, format, HDToolbox : all failed.

I have read that this utilities need SCSIDOS 3.0

Someone can provide me this utility
Or the original disks from C-LTD

Please help me format this beautiful huge hard drive !!!

The hard drive is an Okidata OD540. The box is a standard scsi box with a terminator. This drive has been formated  with success under OS 3.1 by the seller before I plugged it on my A1000.
