We need to narrow down the problem to either the 2091 or the Kickstart. Start by putting your 1.3 chip back in, so that the only change from before the upgrade will have been the addition of the 2091.

Is there any RAM on the 2091? Make sure the RAM jumper is set to the correct size. There are also some chips on the 2091 that can be reseated, so give that a try (carefully). The errors you're describing don't sound like they could be termination related, but check that as well.

SCSI termination. I think the board has termination onboard, but the other end of the drive chain needs to be terminated as well. Maybe your working drive is terminated and the other isn't?
