Did you set the automount flag in HDToolbox? Try issuing a diskchange command to the volume (DH0:, etc). It might show up then. Or try formatting from the command line.

Having re-read your post, you might need to adjust the jumpers on your drive. I noticed a dramatic increase in the sensitivity of the SCSI controller when I updated the firmware on my board. Start by turning on term power. What other jumpers are there?

But first, see if there's anything odd in the Cyberstorm boot menu, particularly the SCSI page. Some settings there might have been altered/reset by the firmware upgrade.
