I have been getting this software failure on several occasions but randomly on some programs. But then I was trying to to use STFax 4.5 (http://aminet.net/search?query=stfax) and every time i use that program, I can always get a 0100 000C error.
I'm running 3.1.4 on an A4000T with an 040.
Has anybody else tried STFax 4.5 on a clean install of Amiga OS 3.1 or 3.1.4?
The only way I don't get a guru is when I do a "NoFastMem" before I launch. In that case it works. So I started to think there was something wrong with the A4000T fast memory and started to pull stuff all expansion cards out etc. but even then still got error.

This alert means "memory insane". As it seems, it trashes memory, and writes above the memory bounds it has allocated. There is nothing you can do - inform the author (probably away since a long time - otherwise this would have been your best option).

Thanks Thomas for the reply. I'm hoping it is not some hardware issue as it sounds like a software problem. If I have the chance, I will try STFax on a A3000.

Looks like a software issue to me now for sure. On my A4000T, I was in Amiga's native 4-color mode in Workbench and it failed and when I went to 8-color, I did not see any memory corruption issue. I tried the same on the A3000 and saw same behavior. I'm thinking it is something to do with the MUI class that is being used for the toolbar. The images are not getting loaded properly in the toolbar when in 4-color mode. When it fails, I'm getting blank buttons for most of the images.
