Like new with original box and manual, and I attached the VGA cable to a cover plate to mount properly. FPGA firmware version 1.5. Works great!
I don't believe there are any of these left for sale new, so these are probably getting hard to find. I've attached a picture of it installed in my 4000 just before removing it.

But where goes the vga bracket when every slot is occupied?

You don't have to mount the VGA port in the metal bracket, you can just hang it out the back since the lowest opening is actually wider then the rest for a video toaster. I had my Deneb mounted in that zorro slot with a USB bracket in the same spot and the VGA cable fit out next to it just fine. I later found the nice bracket and moved the Deneb to the top.

The VGA bracket isn't supplied normally with the Indivision, you could always use one of the other blank expansion bays on the back of the A4000.
Also not all Zorro cards make use of the rear slots, i.e Buddha IDE - so you'd still have a spare slot for the VGA bracket if indeed you decided to use it :)
