I have to play with the frequencies and voltage ... 100MHz is a bit unstable.
Sometimes the systems hangs, no matter what type of memory is used.
It happens in chipmem, zorram, fastmem, 32bitmem.
Tested with analyser, after some passes it hangs.
(I think i have to try different corevoltages or the cpu has reached the limit)
For now i have a rockstable 90mhz setup:

higher voltage will solve some problems but not all. It's what the other logic chips can tolorate. great work but it needs to be stable,if you can fix the problem the the card may go faster.
i will soon finish my special A1200 motherboard. hopfully it will fix a few issues im having when PPC accelerator card is overclocked above 83Mhz but would like to point out that my PPC card has booted @86Mhz,but if i remember correcly it also boots @92Mhz.
@100Mhz it will not load workbench.

I like the insides of your 4000D. The ventilation update is very nice looking, and the rest is neat and clean. I had deliberated on getting a 4000Di PCI for my 4000D for a while, but I decided that I would be just as happy with my PIV and a Deneb.
I especially like your MacOS demonstration in this vid... should piss off a few Mac purists :D

What kind of 68060 CPU are you using? With a rev. 6 CPU (E41J mask set) I am able to use it at 100 MHz without higher voltage, but as you write the system will randomly freeze, it can be after minutes or hours of use.

I have a stable set up with a Cyberstorm MK2 at 80Mhz, latest 060 mask too. Would like to try higher frequencies but I doubt it will handle it.
The 80Mhz clock is almost enouph to play quake 1 at 640x480 8bit with software render, but it still stutters too much. 100Mhz must be close to fluid.
I've read somehwere that the CPU boards that were used on A4000Ts (Quickpak or something) are good overclockers too...

How long has your MKII been running that way? I put a Rev 6 in mine along with quality RAM, but I'm hesitant to push it past 66MHz. From what I read (googling/usenet) the MKII was designed to handle 66 and even 80MHz, but I imagine that was intended to be done after a firmware update. At least that was according to Phase5 back in the day.

A firmware update might be usefull, I never messed with that on mine, don't even remember if it has the lastest firmware (almost did but was afraid to ruin a good working card).
It's been running like that for months but to be honest I don't fire up my 4000T that much except for some dev stuff once in a while. When I do it's normally up for 2 to 3 hours but it's not on 100% CPU load.
Good 50ns ram is essential, maybe there's even better.
Also, don't forget that there were MK2's running 040's at 40Mhz, which means they used an 80Mhz clock (040 CPU uses half clock rate), so 80Mhz won't damage the card, just be sure to use an oscillator socket to be able to exchange them easily and keep an eye on CPU temperature.
I really got to try 90Mhz or more :)

From what I read, the firmware updates for higher CPU frequencies never came. Phase5 dropped support of the idea entirely, since Motorola never released 66 or 80MHz (full) '060s. Apparently, the MKII firmware author (Carsten Schlote) left Phase5 shortly after the MKII was released which might have had some impact as well. (The above might be inaccurate, it's just what I read perusing old usenet posts.) Mr. Schlote released an unofficial firmware, but I never tried it because (1), it's AFAIK irreversible, and (2), as you mentioned, fear of a b0rked MKII. I read a few stories concerning the latter. :-/
I think you are right, it shouldn't damage anything to try. I'm guessing the firmware update would have relaxed some RAM timings, which explains why good RAM is required for 80 (and even 66MHz) overclock and the board jumpered for '060 operation. (I don't think the SMD jumpers are of any help here, unlike the Apollos.)

the MK3 is essentially a PPC board without the PPC,so any overclocking that applies to the 060 on a cyberstorm PPC is relevant to the MKIII.

EDIT: From what I recall, Ratte is 100% correct - there's nothing in the firmware that can't be gained using the MMUTools. And AFAIK there's no tool to flash back your original firmware... though I wouldn't mind having a copy just to check out the readme.
