Get CGX4 instead, version 3 is outdated anyway. Vesalia e.g. sells it still.

it seems you have it installed correctly! :)
just got to prefs and run screenprefs
from there choose a cybergraphics screen mode and voila...
workbench on VGA monitor

Did you definatley select CV64 and not the 3D version? I installed it for a CV64 3D by just going through the installer and it all worked fine.

One thing to try -
Boot without startup-sequence, go to sys:prefs/env-archive/sys directory, and delete the screenmode.prefs file. Reboot, WB should start in a native PAL/NTSC screen. Then go to your screenmode prefs, try testing a Cybervision screenmode and see if you get a picture. If all goes well, save the mode and give it another try.

Do you have a monitor pluged into the native Amiga port and the Cybervision 64? you may be getting as error on one of the outputs and your not seeing it?

I had similar issues with a CV64 hanging/freezing during bootup on my A3000. It turned out to be a faulty card, got another one and worked first time. Apparently it is quite a common problem (search the forums here and via search engines) so chances are your card may be to blame.
have you tried the following:
0) Try Picasso 98, an alternative (and for many a better one) to the Cybervision drivers.
1) remove any extra cards / external peripherals and see if it boots then
2) change the resolution to PAL (booting through an alternate startup-sequence) and then to a CV64
3) clean the contacts of the card and the zorro expansion slot. Make sure it makes proper contact.
4) Is your power supply providing enough juice? Use a multimeter to check the 5 and 12 volt lines.
X) if your CV64 is the 4mb model (*only after you've tried all other options*), try physically removing 2mb of memory (I tried this with a tiny screwdriver) from the card and try again.
Good luck and keep us posted on progress!

When going through the install, if you're using passthrough (one monitor plugged into the Cybervision board, and the output of your Amiga's video going to the input of the Cybervision board) be sure to choose the passthrough option that suits that configuration. I've had Cybergrafx logo get stuck when I forgot to enable passthrough.
I've said it before, setting up RTG cards can be a pain.
