I just looked at the Big Book of Amiga Hardware web site. http://www.amiga-hardware.com/
To my surprise, the company I was most familiar with for Amiga 2000/3000/4000 accelerators is not listed in there.
RCS management, located right where I live in Montreal made the Fusion forty accelerator http://amiga.resource.cx/exp/fusion40
It is said that: The Fusion Forty was used in the amigas which have done parallel rendering of images for the TV series 'Babylon 5' at Foundation Imaging.

It is also said that it was the very first 68040 accelerator ever for the A2000.
RCS also did the X-Calibur, the only "accelerator for an accelerator" that stacks on top of the A3640.
