The Boing Ball in the middle was a decision made a long time ago (just like Windows and some Linux keyboards) but if you want I can make one like this:

So, how is the swapping of alt and amiga keys done?

By swapping the two keys around.

If you mean in software...I know that some people use Allkeys to remap them.

You can mount the keys the way you want, I prefer the two "A" at the extremes of the space bar, then map the keys...but that is just me.
