It could be a dead drive, it could be that you haven't got the EIDE99 software setup quite right.

The things in Devs/Dosdrivers are part text file and part icon, not really drivers as such.
EDIT: The reason to try the CD on the secondary chain is to check that devices can be accessed from that socket. You will need to set the Zip drive to slave to test it on the same chain as the hard drive,
IIRC the secondary channel won't work without the EIDE99 software installed, that could be your problem.
EDITEDIT: Elbox aren't the only type of 4 way EIDE adapter.
Most relevant info I have is an arduous labour to try and get a CD-R mounted right on secondary master with 3.9.
