How to determine screenmode ID?

Some applications allow you to manually enter a screenmode ID number into their configuration files. Lightwave Modeller allows this for instance. Once entered into its config file manually it will start up in that screenmode, saving me from having to switch it manually from the GUI.

How can I determine the ID number for a screenmode?

In OS 4.x, it is also displayed in the ScreenMode Prefs program, at least since 2008 (or whichever update was the first after that).
