If you count pinball machines -- they were in arcades, too -- then it would be "PinBot," hands down.  Otherwise, "Last Resort" on the NeoGeo.

Second would be "BallBlazer" on just about any platform: Atari 5200, Commodore 64, Atari 8-bit.  Honorable mentions would be "CyberSled," "Xevious," both arcades, though several home conversions of "Xevious" are quite nice (including the J2ME recently released.)

My favorite game of all time has to be Terranigma on the SNES, it is in my humble opinion the best R.P.G. game ever on any platform... :)

After a whopping 33 years of gaming, here is my analysis.  Best games ever:

1. River Rescue (Vic-20)
2. Ultima V (Amiga)
3. Raiders of the Lost Ark (Atari 2600)

Pro Evolution Soccer.  I like V5.  Wasted too many hours of my life on this.  Only reason I bought a PS2, after accidentally discovering  ISS ProEvo 2 on the PSONE for $2 at EB games.

There are a few that stand out for me:

- Goldeneye (N64). Fantastic longevity thanks to the rewards system and fantastic multiplayer.
- Zelda, Ocarina of Time (N64). Nearly failed me my school leaving exams
- Wipeout 2097 (PSX). Oooooh, the buzz from pulling off a perfect lap and eliminating your main contender.
- F-Zero GX (Gamecube). Different beast altogether to Wipeout, and perhaps more addictive. Looks incredible too!
- Worms: Director's Cut (Amiga). Holds my record for longest gaming session - 13 hours over night with some friends and some beers.

Half Life 2 and Max Payne on the PC, and Joust on the Atari 800 XL.  I had a blast playing all the old Amiga games, but the above games really left an impact on me.

Oh, Circus Atari on the Atari 2600 as well.

It's kinda hard to pick only one game from just one platform over the last 25 years that I have been enjoying games. However, I could form a list of one or two strong contenders. Here it is:

Coin-up machines: Altered Beast

Atari 2600: Millipede

GameBoy: Mortal Kombat

NES: Mario Bros

SNES: Street Fighter

Amiga: Sensible WoS (along with many others!)

PC: Age of Empires, Quake II and III

Dreamcast: Headhunter

On any platform the top 4 would be all FPS.
Doom2 (Listening for network start info (IPX/SPX)
Quake2 (Various Mods)
Counter Strike (original up to version 1.5)
Then BF1942 and especially the Desert Combat mod for it.

On Amiga my fav game was Frontier.... hours of youth in orbit around Saturn.

Hard question to answer after 30 years of playing computer games, but if I absolutely *had* to make a decision I might be inclined to say Deus Ex. I dont recall a game I got so completely absorbed in. Huge game too, so it takes quite some time to complete.

Other mentions must got to both Rtype and the Street Fighter series, both of which I spent a fortune on at my local arcades rather than going to school :-) Unlike a lot of people I enjoyed Street Fighter 1 when it was 1st released. I became fairly decent at it, and would usually end up with an audience, but the real addiction started with SF2 and while I no longer play it daily the series still gets a bit of attention from me.
Near as I can tell this is where I write something under the guise of being innocuous, but really its a pot shot at another persons/peoples choice of Amiga based systems. Unfortunately only I cant see how transparent and petty it makes me look.

Haaard :( OK, I haven't been gaming for 30 years like some of the guys here, but ~18 is a lot, too. By platforms:

Amiga - UFO, Speedball 2
PSX - Metal Gear Solid
PC - System Shock 2, Wizardry 7
Xbox - Morrowind, Fable
PS2 - GoW 1, 2
etc.

I'm sucker for anything with great storytelling...

These days Im the same. While I favored arcade style games when I was younger these days a story I can get into will probably make me delve into a game than any other trait. This worked out pretty well for me actually as I never played many ScummVM games when they were new, so there was plenty of "new" commercial games for me to play on Amiga OS (and later AROS) for me to play as my tastes changed.

This isnt to say point n click adventures are what I exclusively define as a game with a storyline, but they do fit into that category. Although not really known for it's story the original Max Payne started me on this taste for a story, and Deus Ex more or less cemented (as much as my age) a change in direction for my taste in games. Speaking of which I'll have to play it again with the hi res texture packs, gives me an opportunity to play the story out differently this time (there's multiple endings depending on your decisions in the game).

Some quick picks that come to mind:

The entire Jet Set Willy series, and Manic Miner on ZX Spectrum. :-)

Worms 2: Armageddon and Worms World Party on the PC.

Pirates! on C64 (Amiga version was cool too).

Ruff 'n' Tumble on Amiga.

How about on "each" platform? ;)

Commodore Amiga - "James Pond 2: Robocod"
Commodore 64 - "Ghostbusters"
PC - "Carmageddon" (with "Stunts: 4D Sports Racing" a close second)
Mac - "Photoshop"

Atari 2600 - "Yar's Revenge"
Atari Jaguar - "Tempest 2000"
Sega Genesis - "Toejam & Earl"
Sega Saturn - "Baku Baku Animal"
Sega Dreamcast - "Chu Chu Rocket"
Playstation 1 - "Wipeout XL" or "Micro Maniacs" (tie)
Playstation 2 - "Katamari Damacy"
Playstation 3, XBox, XBox 360 - buy me one and I'll tell you.
Nintendo Entertainment System - "Duck Hunt"
Nintendo Wii - "Epic Mickey" (soon.)

Atari Lynx - "Chips Challenge"
Nintendo Gameboy/GBC - "Tetris"
Nintendo GBA - "Tron 2.0" (for the minigames)
Nintendo DS - "Space Invaders Extreme"
Sony PSP - "Echochrome"

but overall? I'd say #1:Tempest 2000, #2:Katamari Damacy, #3:Carmageddon

ET on Atari 2600...
Probably longest time spent on NemacIV and Shogo. And Flatouts on PC.
